Accident Investigation Bureau has released six final reports on plane crashes with twenty four safety recommendations to the regulatory body, affected airlines, airport operators and foreign agencies.
This is to prevent recurrence of similar accidents or serious incidents.
Lilian Eze-Mark reports that human error has been blamed on five of the accidents while one is mechanical.
The six accidents involve Bristow Helicopters’ Sikorsky S76C++ helicopter with registration number at Murtala Muhammed Airport, Lagos in 2016, Boeing MD-83 aircraft operated by Dana Airlines, which occurred at Port Harcourt International Airport in 2018, and another involving an Airbus A330-223 aircraft by Delta Air after takeoff from Runway 18R, Murtala Muhammed International Airport, Lagos in 2018.
Others are Cessna 208B Caravan, belonging to Taraba state government which occurred in Adamawa state in 2012, Diamond DA40D aircraft by International Aviation College, Ilorin, which occurred at Ilorin International Airport Illorin in 2013. And Gulfstream G200 Aircraft by Nestoil PLC, which occurred at Nnamdi Azikiwe International Airport Abuja in 2018.
The causal factors for the Cessna 208B accident include that the pilot was not certified to fly the aircraft and inadequate oversight by the Nigerian Civil Aviation Authority.
For the fire at the tail of engine number one of Delta Airline’s A330-223 aircraft, the agency made safety recommendations to America’s National Transportation Safety Bureau.
With the reports released Accident Investigation Bureau now has forty six final reports and made one hundred and seventy eight safety recommendations but still has twenty two outstanding reports.
